Key question: effective?

Population groups:

  • People with long-term conditions
  • People whose circumstances may make them vulnerable
  • People experiencing poor mental health (including people with dementia).

These examples relate to Key Line of Enquiry (KLOE) E1: Are people’s needs assessed and care and treatment delivered, in line with current legislation, standards and evidence-based guidance?

When we inspected

The practice kept a register for people with learning disabilities. We saw that, of the 72 people registered, approximately 10% had care plans and had physical health checks carried out in the last year.

Why this is inadequate?

This practice was not effectively assessing the needs of this particularly vulnerable group of patients, nor proactively planning care and treatment for them. The health needs of this group can be complex: patients won’t necessarily know how or when to access care, leading to an inevitable inconsistency in the quality of care they receive.
