People do not know or do not understand what is going to happen to them during their care. People do not know who to ask for help. They are not involved in their own care or treatment and neither are carers or family members.

People's preferences and choices are not considered in their care, support and treatment.

The service does not support a caring environment and approach to people's care, treatment and support.
