People’s needs had been assessed and managers and staff worked closely with people’s relatives and health professionals to ensure care and support was effective. People had health care plans in place, but these lacked detail in some areas where the person had complex health needs. Consent was documented and the provider monitored people’s outcomes through regular review meetings.

Assessing needs

People told us and relatives confirmed they had been involved in the development and review of their care plan and risk assessments. One person told us they had a weekly meeting with the registered manager to discuss their current care record and whether this required updating. This person told us they had found this meeting very helpful.

People were involved in the assessment of their needs and in the development of their person-centred plan. People’s communication needs were assessed to ensure staff could support them in the best way for them to be involved in their care.

People’s assessments were reviewed regularly by a manager to ensure they remained relevant and to document and share any changes. In some cases, these reviews were shared with social workers in accordance with their contractual obligations.

Delivering evidence-based care and treatment

People told us and relatives confirmed they had been involved in the development and review of their care plan and risk assessments. People’s nutrition and hydration needs had been recorded within their care plan. However, concerns had been raised regarding staff’s knowledge in relation to their cooking skills.

Staff provided care that was based on current good practice and relevant to the person’s needs. For example, one person was receiving support to maintain a healthy diet; the person had been fully involved in the planning process and understood reasons. This was reflected in their care plan.

The provider worked closely with health professionals to ensure the care and support they provided was based on evidence and best practice guidelines.

Staff teams worked well together on care calls that required two staff members. Staff had received specialised training from the community nursing teams where required.

Supporting people to live healthier lives

People told us and relatives confirmed they had been involved in the development and review of their care plan and risk assessments. Information regarding support the person received from external healthcare professionals had been recorded within their care plan.

Risks to people’s health care needs were not always made clear in care plans. For example, epilepsy care plans needed more detail about the potential risks that could be associated with epilepsy. However, epilepsy plans did include details about triggers, seizure presentation, when to seek further assistance and seizure monitoring charts. The registered manager took immediate action and reviewed people's care records and updated them where necessary to ensure they contained the relevant information to enable staff to keep people safe. People were encouraged to make healthy lifestyle choices, for example, in relation to diet and exercise.

People told us and relatives confirmed they had been involved in the development and review of their care plan and risk assessments. People told us and relatives confirmed their views were sought via review meetings and quality assurance visits however, one person told us they had raised concerns that had not been acted on in relation to care staff not following their care plan.

People’s care records contained details about expected outcomes, goals and aims. These were reviewed regularly to ensure they were still fit for purpose and that goals were being achieved. For example, one person has been supported to get a part-time job.

The provider had processes in place to monitor people’s care and treatment. The management team reviewed and audited care records regularly to ensure the care and support provided was in line with plans.

People told us and relatives confirmed that staff sought people’s consent before any care or support task.

Staff told us how they always asked people’s consent before carrying out any care or support task. People’s views were taken into account when developing their care plans. People were supported to make informed decisions.

Assessments and care plans had documented people’s consent to receive care and support, and detailed how consent was to be gained. People’s mental capacity to consent to care had been assessed.
