The hospice worked closely with the local NHS trust’s palliative care team and accepted referrals from GPs and other health and social care providers. They operated urgent access and rapid discharge pathways, which were responsive to patients' needs and risks. Staff had access to communication aids to help patients become partners in their care and treatment. Managers and staff ensured that patients did not stay longer than necessary. Service provision was discussed at daily multi-disciplinary meetings, which included plans for appropriate discharges to patients' preferred places of care. Discharge planning was especially careful for those with complex mental health and social care needs. Staff interactions with patients allowed ample time for questions, clarity, and understanding of treatment and care. Patients and families reported that staff were kind, considerate, and very caring throughout their care pathways. Staff had plenty of time to spend with patients, fostering strong relationships, as reflected in patient comments. Managers planned and organised services to meet the needs of the local population, working with others in the wider system and local organisations to plan care. They tailored services to individual needs, ensuring flexibility, choice, and continuity of care. Feedback from staff indicated they received insights from incident investigations, involving patients and their families in these processes. Inpatient unit staff understood the support available from other agencies and followed established referral processes. When mental capacity assessments showed, patients could not consent to care and treatment, staff made decisions in the patients' best interests, considering their wishes, culture, and traditions.

Daily multi-disciplinary meetings facilitated comprehensive care planning and review.
