We found staff to be open to discussion and feedback. The provider demonstrated a transparent and open culture in relation to people’s safety. Staff told us leadership was effective and there was an emphasis on people’s safety and continually striving to improve. Staff told us they had clear responsibilities, roles and systems of accountability to support good governance and management. Staff told us improvements had been made to ensure they felt respected, supported and valued. We saw the practice had implemented effective processes to support and develop staff with additional roles and responsibilities.

Systems and processes were embedded, and staff worked together in such a way that the assessment did not highlight any issues or omissions. The information and evidence presented during the assessment was clear and well documented. The practice had a governance system which included policies, protocols and procedures that were accessible to all members of staff and were reviewed on a regular basis. We saw there were clear and effective processes for identifying and managing risks, issues and performance. Systems to reduce the risk of Legionella or other bacteria developing in water systems were effective. Practice protocols had been updated to reduce the risk of sharps injury. Systems were in place to ensure all staff received appropriate Disclosure and Barring Service recruitment checks. Processes were in place to ensure effective risk management in respect of staff Hepatitis B immunity. Improvements had been made to ensure leaders sought and acted on feedback from staff. Staff stated systems had been reviewed to ensure feedback from staff was acted upon with a view to improve the quality of the service. The practice had also made further improvements: Systems were in place for the appropriate and safe handling of medicines. The emergency medical kit accurately reflected current guidance. Protocols were implemented for the use of closed-circuit television cameras taking into account the guidelines published by the Information Commissioner's Office. Systems were in place to ensure patient referrals to other dental or health care professionals were centrally monitored. Audits of infection prevention and control were undertaken at regular intervals to improve the quality of the service. The most recent audit showed that, where appropriate, audits had documented learning points and the resulting improvements could be demonstrated.
