We identified 1 breach of legal regulations in relation to governance arrangements. The provider failed to have effective governance arrangements in place to monitor quality in the service. The provider failed to maintain contemporaneous records in relation to people’s care.

Capable, compassionate and inclusive leaders
Leaders lacked the skills and knowledge to lead effectively. The leadership team failed to identify the concerns found during the assessment and ensure they always met legislative requirements. For example, in relation to medicines management and health and safety legislation. Due to the concerns identified, we took urgent action to ensure people's safety. The provider was responsive to our findings and worked promptly to address our concerns.

Governance, management and sustainability
The provider failed to have proper processes in place to monitor the quality and safety of the service. Risks to people using the service were not always identified. We found several examples where records were missing or incomplete. Although the provider had a quality assurance policy in place, it failed to provide clear guidance for staff on how to implement, monitor, and develop any quality assurance systems. The registered manager told us they carried out a number of quality checks in the service with the support of another director, however evidence of these checks was not always available to us at the time of the assessment. Following our assessment, the provider produced an action plan to begin to address the concerns found during the assessment. We will check for improvements at our next assessment.

Learning, improvement and innovation
The provider did not have clear processes in place to learn lessons. The provider had developed an action plan following a local authority visit which identified actions taken to address shortfalls, this document rated most items as complete, however the areas highlighted during this inspection indicated these actions were not complete. Regular staff meetings did take place were people’s needs and any changes were discussed and communicated to the team. However, this did not always result in people’s care plans being updated to reflect any changes.
