Acacia Lodge is a residential care home providing accommodation and personal care to a range of people, some of whom have dementia, mental health or physical needs. The home also provides a respite service. The service can support up to 32 people. At the time of the assessment there were 23 people living at the service. We carried out our on-site assessment on 17 April 2024, off-site activity started on 17 April 2024 and ended on 14 June 2024. We assessed quality statements from the safe, effective, caring, responsive and well-led key questions and found areas of concern and some good practice. The scores for these areas have been combined with scores based on the key question ratings from the last inspection. We looked at 15 quality statements. Learning Culture, Safeguarding, Involving people to manage risks, Safe and effective staffing and Medicines optimisation. Delivering evidence-based care and treatment, Supporting people to live healthier lives and Consent to care and treatment. Independence, choice and control. Person-centred care, Listening to and involving people, Equity in experiences and outcomes and Planning for the future. Freedom to speak up and Governance and assurance. At our last visit to this service on 10 May 2022 we found there were not robust systems in place to meet people’s nutritional and hydration needs and maintain good governance at the service. At this assessment we found the provider still needed to make improvements in these and other areas of the service. We found five breaches of regulations in relation to person-centred care, safe care and treatment, meeting nutritional and hydration needs, good governance and staffing. We have asked the provider for an action plan in response to some of the concerns found at this assessment.

People's experience of the service

People said staff were caring and supportive. Staff knew their care needs and preferences. Most people said they could raise issues when they needed and were listened to and found the manager approachable. There were enough staff to support people to be safe. The service worked with healthcare services to meet people’s health needs. Some people wanted more support to enjoy more meaningful activities. However, our assessment found areas of the service did not meet the expected standards. Risks to people’s safety and well-being were not always managed appropriately and people’s medicines support was not always managed safely. Staff did not use the service’s digital care systems appropriately and records about people’s care were not always maintained and their information responded to effectively. People’s care plans were not always up to date as they did not always recognise and reflect their individual needs and personal preferences, such as for end of life care. The quality assurance systems had not enabled the provider to identify and take timely action to address these areas of concern.
