The provider had systems and processes to keep clinicians up to date with current evidence-based practice. We found patients’ immediate and ongoing needs were fully assessed, these included interventions required to ensure effective care was delivered.

Assessing needs
Patient feedback about their experience of assessing needs at the practice was positive. People were involved in the assessment of their needs, and support was provided where needed to maximise their involvement. People were confident that their individual needs had been appropriately assessed and were fully understood.
People’s needs were assessed using a range of assessment tools to ensure their needs were represented and understood. The needs of carers of people using services were also assessed and met.
The provider had systems and processes to keep clinicians up to date with current evidence-based practice. Patients’ immediate and ongoing needs were fully assessed. This included their clinical needs and their mental and physical wellbeing.
