People’s care and treatment was maximised because the provider assessed and reviewed people's health, care, wellbeing and communication needs with them. The provider worked effectively across teams and services to support people were supported to manage their health and wellbeing so they could maximise their independence, choice and control. People were supported to live healthier lives and to improve care and support outcomes. The registered manager and management team routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent. People are aware of their rights around consent and staff respect these when they deliver person-centred care and treatment.

People and relatives told us their needs had been assessed before moving into the service and staff understood their needs.

Staff knew about people’s health and social needs and preferences and gave them as much choice and control as possible.

People's needs were assessed before they started using the service to ensure staff understood their needs and preferences. Care plans contained information about people's religious beliefs and their personal relationships with their circle of support.

Delivering evidence-based care and treatment

People told us they were involved in ensuring they were receiving care in line with their goals.People received good support with eating and drinking.

Staff were aware of the need to involve people in all aspects of their care and ensuring they were meeting their assessed needs. Staff knew each person's dietary needs including their allergies, religious preferences and consistency of food they could eat safely

The service had evidenced based assessment tools embedded in their care planning system, for example, tools related to nutrition and skin integrity. These helped to ensure care was planned in line with current best practice guidance.
