Well-led
In this Key question we looked at 2 quality statements. Governance management and Freedom to speak up. We found one breach in the legal regulations relating to governance. We found governance systems and audits were not effective in identifying or addressing areas for improvement We found the Registered Manager had created a positive culture where staff feel confident their voices will be heard through having an open-door policy in the home. Staff said they are given regular opportunities to speak up.

Freedom to speak up
We saw processes are in place for staff to feedback, through supervisions, team meetings and a staff suggestion box. However, we did not see evidence of discussions held with staff in team meetings where shortfalls in practice were identified by the Registered Manager. For example; where shortfalls have been identified room check audits had fallen below standard in December 23, staff are not asked why they thought this was happening.
The Registered Manager said they have created a positive culture where staff felt confident their voices will be heard through having an open-door policy in the home. Staff told us; “We are given regular opportunities to speak up. We are asked in supervision.” “Management are very good at listening to us.” " I feel confident to speak with manager. Last year I asked for training to take blood and straight away this was done."

Governance, management and sustainability
The Registered Manager said they had effective oversight of quality assurance audits and completes sample checks audits to check they are completed. The Registered Manager ensured staff understand of roles and responsibilities through induction, training and people's job description. The Registered Manager said they completed regular sample checks of staff practice and knowledge though observations. One staff we spoke with confirmed they were aware of their responsibilities and felt confident with their responsibilities in the home and said their manager checks their work.
We identified one breach of the legal regulation in relation to governance. We found the provider has continued to fail to establish and operate governance systems effectively. We found concerns, errors, and omissions identified during our assessment had not been identified by the provider and the provider's oversight and audit of the home were not effective. For example, we found reports from provider visit records were not structured and did not identify actions to take where shortfalls were found.
