Safe
We found the provider had implemented effective measures to address the issues we had identified during our previous inspection. We found that learning was shared from safety incidents and implemented successfully to improve patient care.

Learning culture
At our previous inspection we identified a serious incident that had not been reported or investigated appropriately. During this inspection we noted the following improvements: We were provided with a comprehensive ‘After Action Review’ of the incident concerned. This provided a detailed analysis of the event, including what had happened, what could be expected and identified key areas of learning. The review had been undertaken by a number of senior managers within NHFT and included a vascular surgeon from Milton Keynes Hospital to ensure that all aspects of the incident were considered. As a result of this review, a decision had been made to implement datix reporting for all unplanned patient admissions to hospital. We reviewed 6 recent hospital admissions and found corresponding datix reports had been completed. All staff we spoke with were aware of this new policy and confirmed it was being implemented successfully.
