Services are not delivered in a way that focuses on people’s holistic needs.
There is some flexibility to take account of individual needs as they arise, but the service does not meet the needs of all the people who use it.
Services are delivered in a way or at a time that is inconvenient and disruptive to people’s lives.

People find it hard to access services because the facilities and premises are not appropriate for the services being provided and action is not taken to address this.
