We assessed one quality statement in the effective key question and found areas of good practice. The scores for these areas have been combined with scores based on the rating from the last inspection, which was good. Our rating for this key question remains good. People's care plans demonstrated their needs had been assessed and expected outcomes were identified. Care and support was regularly reviewed and appropriate referrals were made to ensure people’s needs were met and the best possible outcomes achieved. Staff described how they ensured people were involved in the planning of their care and worked hard to deliver care in a way that was important to them. However, we identified some improvement was required to ensure records of the care provided were consistent with people's assessed needs.

Delivering evidence-based care and treatment

People's feedback confirmed care and support was planned and delivered according to their needs. People told us staff supported them to access external health professionals when required. Comments included, "They’ve [staff] sorted transport when I’ve needed to go for a CT scan, I’m hard to please but they are good at arranging things and they check on me a lot" and "They are good as I have to go to the hospital three times per week so they sort that out for me." People provided positive feedback about the quality of food. Comments included, "We can have fish and chips on Friday, it’s my favourite" and “The chef is very good here, lovely food.”

Staff described how they ensured people were involved in the planning of their care and worked hard to deliver care in a way that was important to them. Staff explained they used the information recorded in people's care plans to get a basic understanding of their needs, but then they were able to spend more time with them to fully understand their needs and preferences. A staff member told us, "We have 'who am I ' which is very well recorded, so we get a good overview of people and then of course we can spend time with them and chatting to them about their care and support."

Care and support was regularly reviewed and appropriate referrals were made to ensure people’s needs were met and the best possible outcomes achieved. Although we were assured people achieved good outcomes through effective care, treatment and support, we found people's daily care records were not always properly maintained. For example, for a person who required regular repositioning to reduce risks associated with their skin condition, records did not always reflect this had been done in line with the frequency outlined in their care plan. For another person, records did not reflect the amount of fluids consumed was consistent with their daily target. We shared our findings with the registered manager who confirmed staff were regularly reminded about the importance of keeping up to date and complete records and demonstrated the process they had put in place to offer additional support to staff in this area.
