Delivering evidence-based care and treatment

People told us they had care and support that was well co-ordinated, where everyone worked well together and with them to provide the care needed. Peoples requests and preferences were respected by staff and they were supported to make referrals for additional support from other agencies if needed. In addition, staff supported people to access information and advice about health, care and support if this was requested.

Staff and leaders told us they received regular training and updates to ensure they are up to date with the national legislation, evidence-based good practice and required standards.

We saw evidence of training and staff updates to ensure that the care is provided in line with the national legislation, evidence-based good practice and required standards. People had care plans that reflected their uniqueness, life story, skills and goals.

How staff, teams and services work together

Staff told us they access to information they needed to appropriately assess, plan and deliver people’s care, treatment and support. Staff told us they had enough time to read and understand people's care plans and were informed when people's needs changed.

We received a positive feedback from the Local Authority about the way the provider worked with them and shared relevant information. We saw examples of referrals to other services, evidencing that support was well co-coordinated.

The provider had processes in place to ensure that people were supported with making relevant referrals when needed. People's needs were considered and assessed as soon as they started using the service and this was clearly recorded. The care staff worked collaboratively with other healthcare professionals to assess, plan and deliver people's care and treatment and to understand and meet people's needs.
