Assessing needs

One relative said their relative was in control of their own care plan and another relative said, “They went through the care plan with me, they were really good and proactive.”

Leaders said currently relatives were not involved with reviewing care plans and risk assessments. They said they had a plan to accommodate this in the next 6 months. The provider said social workers, people and families have been involved in care planning and assessments when planning for a new person to be admitted into the home. The provider said they had developed their own assessment for new residents to cover all aspects of people’s needs. We heard mixed feedback from staff about how they get involved with the assessment and planning for new people being admitted to the home. The provider told us management carry out assessments and discuss the assessment with nursing staff, however we heard that the home's nurses were not always involved with people’s initial assessments. Leaders told us management carry out assessments as having nurses carry out the assessments would leave the home understaffed.

The provider told us the home assessment processes included, National Early Warning Score (NEWS), Waterlow Score, Mental Capacity assessments. The provider told us they complete their own pre-admission assessments for new people are moving into the home, however we found some pre-admission assessments were not always fully completed. For example, in 1 person’s assessment, there was no information identifying if the person required support with their oral health or foot care, and there was limited information around the same person’s pressure care needs. Risk assessments and care plans were reviewed. However, the reviewing process did not appear to be effective. For example, they had not identified the gaps in information identified in this assessment. We found mental capacity assessments had been completed, however, these were not always completed in line with Mental Capacity Act best practice. For example, we saw 1 document included 2 different decisions to be made.
