We assessed 1 quality statement in the effective key question and found no areas of concern. The scores for these areas have been combined with scores based on the rating from the last inspection which was rated good. Our rating for the key question has not changed and remains good. The service was effective. A clear system was in place for the assessment of and continual monitoring of people’s needs and wishes. People told us they felt part of the assessment process.

Assessing needs

People told us they were involved in the completion of their assessments and care plans. Comments included, “Yes, I told them how I want my care given, told them what I needed doing. Yes, (care plan) it’s up to date, it was updated at my last review not long ago. I’d let them know if I wanted to change it, they do ask”; “Yes, they came out and we talked about what help I need from them. My daughter has (care plan) and it’s fine. Copy held at (name) home." The person confirmed the care plan included the care and support they needed. Other people confirmed they did not have a copy of their care plan in their home. However, family member confirmed they had access to the electronic care records system and where happy with their relatives care plan.

Staff and the management team described how they involved people in the assessment and ongoing review of their needs. Staff told us they had access to people's care plans and they were kept up to date. Staff and leaders were knowledgeable about people's care and support needs.

Processes were in place for the initial assessment of people’s needs. This involved staff speaking with people to gather information about their needs, preferences and wishes. In addition, where available, information from other agencies was gathered to assist in the planning of people’s care. Information gathered during the assessment process had been implemented into people’s care plans.
