Monitoring and improving outcomes
Patients told us staff worked with them to ensure they could be discharged safely. For example, making sure they were able to do everyday tasks like washing and making a hot drink. Patients knew the contents of and had agreed the aim of their care plans. Nursing staff benchmarked the care provided to patients against services who offer similar care and treatment, to identify any improvements needed within the hospitals. Staff told us they used the clinical frailty scale, a national recognised tool, to measure patients progress. Meetings took place three times a day to review patients, escalate concerns and plan discharges.
