Care provision, Integration and continuity
Patients were involved in decisions about their care and where appropriate families were also involved. A review of all patients was undertaken prior to their admission to ensure the service could meet their needs. An NHS approved electronic system was used to speed up the assessment process Staff monitored patients and referred them to the acute hospital if their needs changed and the service was unable to continue to support them. Staff and leaders had a good relationship with the local authorities and worked with them when planning discharges.
